The blood and lymph systems are so far the recognized mechanisms through which the cancer cells are transferred to different body tissues. At the present, there are over one hundred types of cancer that have been documented. This paper takes a close examination on the approach to the care of cancer, which consists of diagnosis and staging, the complications of cancer, as well as the available modalities tha t can be used to reduce the physical and psychological effects of cancer and its treatment.Diagnosis and StagingJust like any other disease, the ability to provide optimal care for patients suffering from cancer depends on the offering of well-timed and precise prognosis. Even if a patient is determined to be suffering from cancer, doctors always use biopsy and tissue examination as the final diagnosis method. In the recent past, there have been emerging technologies that are being used by pathologists to determine the presence of cancer with positron emission tomography (PET) and F-fluorodeoxyglucose (FDG) being the most common (DeVita, Rosenberg, 2001).Once the doctor has ascertained the presence of cancer in a certain cell, the next most important stage before treatment can commence is staging. Staging is the simplest way of ascertaining the level of cancer and its exact placement. Staging defines the severity of a personà ¢Ã¢â ¬s cancer on the basis of the primary tumor, as well as on the level that the cancer has spread in the body. This is a critical stage since it makes it possible for doctors to create a prognosis, as well as to develop an ideal treatment modality for the patient. Comprehending the stage and ext ent of cancer is, therefore, one of the most important steps in the process of identifying clinical trials that are appropriate for individual patients. This last type of staging helps in determining the best modality of treating recurring cancer (Johnson, Gross, 2011).Cancer Complications and Side EffectsCancer patients are at greater risk of developing complications either due to the cancer itself or due to the available treatment options. Hyponatremia is the commonest of all the complications relating to cancer. Chemotherapy, which is the most common type of cancer treatment, is considered to be among the risk factors for this complication. This is mostly caused by the syndrome of inappropriate production of antidiuretic hormone (SIADH), which may arise as a result of the ectopic creation of arginine vasopressin (AVP) by the cancerous tissue. Although hyponatremia might be caused by other chronic diseases, research shows that more than half of all the cases are caused by cancer. All the hyponatremia cases that are linked to cancer have been found to hinder t he patientà ¢Ã¢â ¬s response to treatment as recovery. This mode of treatment deals with hyponatremia by openly obstructing the AVP biding with its receptors. Some of the drugs that are recommended for use in dealing with this condition include conivaptan, lixivaptan, and tolvaptan among others (Rubin, Williams, 2010).According to Bucholtz (2010), spinal cord compression (SCC) is also prevalent among cancer patients. It is estimated that more than 20,000 Americans suffering from cancer develop SCC each year. This number is estimated to be on the increase due to the enhanced treatment and the elongated survival in different cancers. This condition is mostly prevalent among patients with lung, breast, prostate, and kidney types of cancer. This complication is mostly brought about by the growth of metastatic tumor that has blocked the epidural space. The unhindered growth of the mass destroys the bony structure of the vertebrae and makes them feeble to the point where they collaps e. If not treated, this injury might lead to a permanent loss of neurologic function. Most clinicians today recommend the use of radiation therapy for the effective treatment of this complication. Before Clegg kidnapped Miranda, he never maintained power over anyone especially the people he cares about, as his mother abandoned him, and both his father and dear uncle died. Cleggââ¬â¢s former powerlessness seems to have induced him to desire a higher level of power that he obtains by capturing Miranda. He is able to abduct Miranda and posses her physical power due to the fact that he wins an immense sum of money, allowing him to purchase a house, thus the ability to maintain her. Clegg thereby controls every physical aspect of Mirandaââ¬â¢s life; when she can shower, eat, go outside, how she lives her life, and even how she dies. This blatant aspect of control that Clegg hordes, entitles him to feel connected with Miranda, furthermore in possession of her. Along with his physical power, he also gains economic power over her. He believes he has a chance to wi n over her love by buying her whatever she desires ranging from expensive food delicacies to artwork and supplies. He employs his economic power over Miranda as a tool to magically make her fall in love with him, only further demonstrating the lack of sanity. Clegg feeds off of the economic and physical power he holds over Miranda, feeling entitled to a position of power that has been absent from his entire life. Moreover, he begins to feel heroic for being able to suffice Miranda with whatever she desires. Miranda begins to seem subordinate, powerless to Clegg, as she constantly has to ask him when she can have a bath, when she can go outside, and overall becomes dependent on him. The fact that Miranda must turn to Clegg for all the vital necessities of life, ensure Clegg that he has complete control over her. Perhaps, fulfilling a life long sense of thirst to have unabridged control of his environment, and the life it involves. Especially being able to capture and control beauty a s he does with his study of lepidoptery, seeing Miranda as a beautiful goddess in the form that all of society desire to posses. All though Clegg possesses unquestionable economic and physical power over Miranda, she possesses powerful beauty both verbally and physically that has an inordinate impact on Clegg. Mirandaââ¬â¢s beauty can be interpreted by the viewer as a sense of power that she holds over Clegg as it lingers over his mindset like a controlling fog. In the sense that even when Miranda speaks to Clegg in a disgusting manner or acts exactly as he despises, he subconsciously excuses her putrid actions as he literally sees her as an enchanting personification of perfection. For example, ââ¬Å"even when she did things considered ugly, like yawning or stretching, she made it seem pretty. The truth was she couldnââ¬â¢t do ugly things. She was too beautiful.â⬠(The Collector, Page 48.) This quote may be more enlightening to the reader than what comes to mind at first as it applies to the overall relationship between Clegg and Miranda. The syntax of this salient quote describes ââ¬Ëthe truth ââ¬â¢ as her inability to do ugly things, which gives the reader insight into Cleggââ¬â¢s perception of Miranda. Possibly interpreted as Clegg feeling eternally inferior to her as he recognizes her immaculate beauty, but controversially that ââ¬Å"â⬠¦he doesnââ¬â¢t have what it is girls look forâ⬠(The Collector, page 11). Yet, his entirely consuming infatuation with Miranda continues to flourish. In addition, Miranda is easily able to express her emotions and thoughts through ardent language, especially her opinions on Cleggââ¬â¢s ââ¬Å"killing of beautiful lifeâ⬠. Miranda despises Cleggââ¬â¢s lack of inner-depth of life. Since Miranda has a high level of education and very keenly observes Clegg, she uses her articulate verbal strength to deprecate Clegg, abating his sense of esteem and thereby inner confidence and power. His character is very weak in the sense that he easily conforms to Mirandaââ¬â¢s beliefs and manipulation. Clegg lacks the abili ty to express his emotions or thoughts verbally, particularly without offending his beloved Miranda. It seems as if he instead tends to stay quiet, constituting Mirandaââ¬â¢s words and beliefs as his own, even when she belittles him. Thus Cleggââ¬â¢s lack of confidence or verbal power to express himself antithetically conflicts with Mirandaââ¬â¢s character that has an eloquent, educated manner of speech that empowers her to take jurisdiction in any of their interactions. In spite of the fact that Miranda has a manipulative verbal power over Clegg, she struggles immensely near the end of the book to maintain this power, which she relied upon as a latent technique of escape. Cleggââ¬â¢s lack of reality and sanity is continuously depicted through out the novel becoming a plausible asset to Mirandaââ¬â¢s escape ideas. The reader understands that Clegg does not have a grip on reality as he is built upon euphemisms, claiming Miranda is ââ¬Ëhis guestââ¬â¢ instead of realistically his prisoner. He thereby abates the severity of the situation he has put Miranda in. Cleggââ¬â¢s oblivious perspective on the truth of his and Mirandaââ¬â¢s relationship is a weakness on his part that entitles Miranda to a certain power. She is able to seduce him to believe he stands a chance to win her love, due to his illogical perspective. This seduction hopefully perchance leading to an action of good morality from his side of ââ¬Å"the manyâ⬠as John Fo wles would say, by liberating her. When Miranda fails to be set free by her utilization of Cleggââ¬â¢s irrationality to become his ideal ââ¬Ëbutterflyââ¬â¢, she approaches a vastly different method of manipulation. She mistakenly tries to sexually seduce Clegg right before she becomes dangerously ill. This act on Mirandaââ¬â¢s behalf caused a momentous change in the battle for power within the novel. Clegg now sees her as ââ¬Å"no better than a common street womanâ⬠as he comes to the realization that she is just like any other foolish, ordinary woman. Who fits perfectly in his narrowly perceived category of desperate women utilizing their bodies much like a whore to achieve what they want. As The Verbal Struggle for Power stated; ââ¬Å"The way Clegg talks to her and treats her from that moment changes. â⬠¦She can feel that she has no longer the power she had before. Now Clegg is more self-confident, he can say ââ¬Ënoââ¬â¢ to Miranda, which he almost cou ld not do earlier.â⬠As soon as Miranda offers herself to Clegg sexually, she loses all the power she inflicted on him before as he feels she is subordinate to him. This decisive act on Cleggââ¬â¢s part to show supremacy over her, allows him to accumulate exactly the key power he is missing earlier in their relationship. Triggering his harsh tone of language that shows Miranda who is the boss, fulfilling his need to feel overwhelmed by power and control, which he so desperately hunted for his entire life. All in all, the novel portrays how Mirandaââ¬â¢s verbal power over Clegg and her beauty vanishes the minute she mentally gives up and uses what seems to her as the only way out, ââ¬Ësexââ¬â¢. From that moment on Clegg procures the confidence and control that he needed all along to feel empowered. Clegg always proclaimed his fascination with Miranda as love, however it appears that his consequential disgust and denigration of Miranda is not love. That he is happier now that he posses exhaustive control over Miranda, drunk from physical, economic, and verbal power. This assignment is a critical reflection of the personal values and beliefs on caring as a nurse of a situation that was based on a geriatric care ward for a patient with dementia, following Bortons developmental frame work. I am a registered nurse working in the medical ward of a multispecialty hospital and this reflection is about an incident where I perceived an ordinary practice of nursing as extraordinary. This reflective incident is about a 70 years old patient named Harry suffering from dementia and started at 7:30 am when my shift starts. It is common for the elderly to develop diseases like dementia and Alzheimers disease with the increase in life expectancy (Fong et al. 2015). Dementia is a condition where the mental ability gets declined and interferes with the activities of daily life like communicating, thinking and memory. This patient was aged and was suffering from a long time without much improvement in his condition and I was involved in his nursing care. From this situation, I was providing the essentials of nursing care for an elderly patient with dementia as the process was challenging and it is quite difficult to manage the patients without having an appropriate understanding of the disease process (Peters, Villeneuve and Belleville 2014). I made active participation in the caring process by providing a safe environment for the patient and it was very much important to treat them with respect and maintain their dignity (Baborie et al. 2012). Person centered care was adopted for providing care to the patient as it is beneficial for the patients of dementia since they are considered as individuals rather than focusing on their disabilities and illness. It helps to treat the patients with respect and dignity (Brooker and Latham 2015). Harry was happy on my arrival and when I approached him for a bath, he became forgetful and confused and it is necessary for the nurses to supervise the patients of dementia during the process. Although the process went smooth, it got derailed at the end when he was asked to leave the bath as he started shouting with swearing and personal insults as he did not want to get out. From the incident, I developed mixed feelings as I belie ved that a previously happy patient turned agitated and distressed due to my actions and this evoked a feeling of guilt that made me upset. Although I was aware that mood swings are a part of dementia, however, considering his condition of ill health made me sad. However, I was happy that I eased the situation and was able to calm him down. This entire situation was ordinary from the perspectives of a registered nurse as they are used to the aggression of the patients of dementia, however, my perception and learning from the incident made it extraordinary for me. When Harry was informed about the changes in his behavior after he was pacified and how I handled the situation, he perceived the situation as extraordinary as well. Although it was my duty to handle such situations with care and kindness, he was surprised how someone can be kind when the patient is going rough and abusive. This was the perception of Harry of my patient centered care for his dementia and he perceived my ord inary nursing care as extraordinary. The outcome of this perception was beneficial for the treatment as he co-operated with me in his other care processes. During my process of pacifying Harry, I had a number of things going on in my mind. I was trying to apply the knowledge and theory of nursing care to get over this embarrassing situation created by Harry during his bath. His reactions were based on the thought that he will be allowed to do whatever he wish without any restriction and my actions were based on the scientific and ethical grounds of nursing care. On stopping him, he outburst with anger and this somewhat frightened and shocked me. However, the reaction was handled with care and I was there with him to calm him down as it was my professional and ethical obligation not to abandon the patient during the process of nursing care (van der Steen et al. 2013). According to the person-centered nursing framework, I was expected to provide holistic care and engagement to the patient and this was the reason I could not leave the patient on his time of discomfort. I was aware of the knowledge of the nursing care for the patient with d ementia and in such cases of patient anger, the most effective way to handle the situation is redirection of the topic in a firm and calm manner (Borson et al. 2013). This will ensure patient safety and reduce his confusion of expression of thoughts and following instructions. However, distracting his attention towards some other direction with the help of playful activities could have served the purpose effectively and made the situation better (Baborie et al. 2012). From the situation, I understood that even trivial events could bring about sudden changes in the mood of the patient with dementia that can even lead to aggression that has to be handled with appropriate patience and care, without leaving the patient. The broader issue that needs to be considered in this incident to make it successful is based on the critical analysis of why and where it went wrong that caused Harry to get angry Patients with dementia needs to be communicated in a way that makes them comfortable, based on the principles of person centered care of nursing (Borson et al. 2013). During his bathing process, if I communicated with Harry with small talks to distract him, the embarrassment could have been avoided (Catic et al. 2014). The consequence of this action could have made him feel ease with the situation and he could have avoided the verbal aggression (van der Steen et al. 2013). From this incident, I learned this skill which I will implement in my future cases of nursing care for the patients with dementia. I also realized that the curricula of the nursing schools should expand to include the courses on gerontology and geriatrics for providing specialized training for preparing the future nurses to provide optim al care to the patients with dementia and avoid such embarrassing situation to confuse and aggravate the patients. By reflecting on this incident, I have identified the gaps that I had in my knowledge regarding the nursing care of the patients with dementia that I bridged as the treatment progressed. Using Bortons developmental framework through action and reflection enabled me to develop the knowledge that is required in the practice of nursing (Rolfe, Freshwater and Jasper 2001). Action and reflection as the core values enabled me as a nurse to work with the patients suffering from mental health and support their families by enabling mutual learning and co-production of the knowledge (Rolfe, Freshwater and Jasper 2001). Mr. Haly was a man of worth, a man of real and substantial merit. He is therefore deeply, and justly regretted by his friends; he was chosen to be a future guardian, and companion for me, and was, therefore, beloved by mine. As their choice as a good man, and a faithful friend, I esteemed him. But no one acquainted with the disparity of our tempers and dispositions, our views and designs, can suppose my heart much engaged in the alliance. (Foster 807) Being similar to Major Stanford in her temptations, even he had to admit the conflict of her inner identity with the society: Her sagacious friends have undoubtedly given her a detail of my vices. If therefore, my past conduct has been repugnant to her notions of propriety, why does she act consistently, and refuse at once to associate with a man whose character she cannot esteem? (Foster 863). In her grief for losing husband, she was not looking for consolation, but was realizing the fact of her ultimately being free of those connections. Moreover, she was seeking consolation not among her friends, but in the fact of being free to choose; in this she was completely different from her female friends, and was constantly judged and warned against Major Stanford. Of course, she was seduced by him, and he never realized the seriousness of his claims on Eliza. She in her turn did not understand that she was misled, and had to abandon her traditional pathway of virtue (so valued in the then society) to be with that man. It was not that Foster was devaluing Eliza, but she was trying to remain within the limits of the traditional society; she could not allow herself openly declaring the fact of Eliza being correct in her moves. This is why the author had to use implicit tools and literary devices for us to understand the true meaning of her writing. The visible devaluation of Eliza was actually the expression of her strength against the widely accepted (and often absurd) norms. In one of her letters, Eliza was writing: Should it ever be my fate to wear the hymenical chain, may I be thus united! The purest and most ardent affection, the greatest consonance of taste and disposition, and the most congenial virtue and wishes distinguish this lovely couple (Foster 813). These were Elizas ideas about her happy union with some man. And while come critics might claim that the image of a woman in early American literature was at least neglected, it can be easily argued. What Foster was aiming was to display the societal constraints in which the woman of her time had to exist. For Eliza this hardly looked like existence, being similar to surviving. In the Other Corner: The Mamluk Dynasty of Egypt While the Mongols were busy with their ever-expanding empire, the Islamic world was fighting off Christian Crusaders from Europe. The great Muslim general Saladin (Salah al-Din) conquered Egypt in 1169, founding the Ayyubid Dynasty. His descendants used increasing numbers of Mamluk soldiers in their internecine struggles for power. The Mamluks were an elite corps of warrior-slaves, mostly from Turkic or Kurdish Central Asia, but also including some Christians from the Caucasus region of south-eastern Europe. Captured and sold as young boys, they were carefully groomed for life as military men. Being a Mamluk became such an honor that some free-born Egyptians reportedly sold their sons into slavery so that they too could become Mamluks. In the tumultuous times surrounding the Seventh Crusade (which led to the capture of King Louis IX of France by the Egyptians), the Mamluks steadily gained power over their civilian rulers. In 1250, the widow of Ayyubid sultan as-Salih Ayyub married a Mamluk, Emir Aybak, who then became sultan. This was the beginning of the Bahri Mamluk Dynasty, which ruled Egypt until 1517. By 1260, when the Mongols began to threaten Egypt, the Bahri Dynasty was on its third Mamluk sultan, Saif ad-Din Qutuz. Ironically, Qutuz was Turkic (probably a Turkmen), and had become a Mamluk after he was captured and sold into slavery by the Ilkhanate Mongols. Prelude to the Show-down Hulagus campaign to subdue the Islamic lands began with an assault on the infamous Assassins or Hashshashin of Persia. A splinter group of the Ismaili Shia sect, the Hashshashin were based out of a cliff-side fortress called the Alamut, or Eagles Nest. On December 15, 1256, the Mongols captured Alamut and destroyed the power of the Hashshashin. Next, Hulagu Khan and the Ilkhanate army launched their assault on the Islamic heartlands proper with a siege on Baghdad, lasting from January 29 to February 10, 1258. At that time, Baghdad was the capital of the Abbasid caliphate (the same dynasty that had battled the Chinese at Talas River in 751), and the center of the Muslim world. The caliph relied on his belief that the other Islamic powers would come to his aid rather than see Baghdad destroyed. Unfortunately for him, that did not happen. When the city fell, the Mongols sacked and destroyed it, slaughtering hundreds of thousands of civilians and burning down the Grand Library of Baghdad. The victors rolled the caliph inside a rug and trampled him to death with their horses. Baghdad, the flower of Islam, was wrecked. This was the fate of any city that resisted the Mongols, according to Genghis Khans own battle plans. In 1260, the Mongols turned their attention to Syria. After only a seven-day siege, Aleppo fell, and some of the population was massacred. Having seen the destruction of Baghdad and Aleppo, Damascus surrendered to the Mongols without a fight. The Mongols approached them, offering an alliance against the Muslims. The Crusaders erstwhile enemies, the Mamluks, also sent emissaries to the Christians offering an alliance against the Mongols. Discerning that the Mongols were a more immediate threat, the Crusader states opted to remain nominally neutral, but agreed to allow the Mamluk armies to pass unhindered through Christian-occupied lands. Hulagu Khan Throws Down the Gauntlet In 1260, Hulagu sent two envoys to Cairo with a threatening letter for the Mamluk sultan. It said, in part: To Qutuz the Mamluk, who fled to escape our swords. You should think of what happened to other countries and submit to us. You have heard how we have conquered a vast empire and have purified the earth of the disorders that tainted it. We have conquered vast areas, massacring all the people. Whither can you flee? What road will you use to escape us? Our horses are swift, our arrows sharp, our swords like thunderbolts, our hearts as hard as the mountains, our soldiers as numerous as the sand. In response, Qutuz had the two ambassadors sliced in half, and set their heads up on the gates of Cairo for all to see. He likely knew that this was the gravest possible insult to the Mongols, who practiced an early form of diplomatic immunity. Fate Intervenes Even as the Mongol emissaries were delivering Hulagus message to Qutuz, Hulagu himself received word that his brother Mongke, the Great Khan, had died. This untimely death set off a succession struggle within the Mongolian royal family. Hulagu had no interest in the Great Khanship himself, but he wanted to see his younger brotherà Kublaià installed as the next Great Khan. However, the leader of the Mongol homeland, Toluis son Arik-Boke, called for a quick council (kuriltai) and had himself named Great Khan. As civil strife broke out between the claimants, Hulagu took the bulk of his army north to Azerbaijan, ready to join in the succession fight if necessary. The Mongolian leader left just 20,000 troops under the command of one of his generals, Ketbuqa, to hold the line in Syria and Palestine. Sensing that this was an opportunity not to be lost, Qutuz immediately gathered an army of roughly equal size and marched for Palestine, intent on crushing the Mongol threat. The Battle of Ayn Jalut On September 3, 1260, the two armies met at theà oasisà of Ayn Jalut (meaning The Eye of Goliath or Goliaths Well), in the Jezreel Valley of Palestine. The Mongols had the advantages of self-confidence and hardier horses, but the Mamluks knew the terrain better and had larger (thus faster) steeds. The Mamluks also deployed an early form of firearm, a sort of hand-held cannon, which frightened the Mongol horses. (This tactic cannot have surprised the Mongol riders themselves too greatly, however, since the Chinese had been usingà gunpowder weaponsà against them for centuries.) Qutuz used a classic Mongol tactic against Ketbuqas troops, and they fell for it. The Mamluks sent out a small portion of their force, which then feigned retreat, drawing the Mongols into an ambush. From the hills, Mamluk warriors poured down on three sides, pinning the Mongols in a withering cross-fire. The Mongols fought back throughout the morning hours, but finally the survivors began to retreat in disorder. Ketbuqa refused to flee in disgrace, and fought on until his horse either stumbled or was shot out from under him. Despite the loss, however, the Mongols did not simply fold their tents and go home. Hulagu returned to Syria in 1262, intent on avenging Ketbuqa. However, Berke Khan of the Golden Horde had converted to Islam, and formed an alliance against his uncle Hulagu. He attacked Hulagus forces, promising revenge for the sacking of Baghdad. Although this war among the khanates drew off much of Hulagus strength, he continued to attack the Mamluks, as did his successors. The Ilkhanate Mongols drove towards Cairo in 1281, 1299, 1300, 1303 and 1312. Their only victory was in 1300, but it proved short-lived. Between each attack, the adversaries engaged in espionage, psychological warfare and alliance-building against one another. Finally, in 1323, as the fractious Mongol Empire began to disintegrate, the Khan of the Ilkhanids sued for a peace agreement with the Mamluks. A Turning-Point in History Why were the Mongols never able to defeat the Mamluks, after mowing through most of the known world? Scholars have suggested a number of answers to this puzzle. It may be simply that the internal strife among different branches of the Mongolian Empire prevented them from ever throwing enough riders against the Egyptians. Possibly, the greater professionalism and more advanced weapons of the Mamluks gave them an edge. (However, the Mongols had defeated other well-organized forces, such as the Song Chinese.) The most likely explanation may be that the environment of the Middle East defeated the Mongols. In order to have fresh horses to ride throughout a day-long battle, and also to have horse milk, meat and blood for sustenance, each Mongol fighter had a string of at least six or eight small horses. Multiplied by even the 20,000 troops that Hulagu left behind as a rear guard before Ayn Jalut, that is well over 100,000 horses. Syria and Palestine are famously parched. In order to provide water and fodder for so many horses, the Mongols had to press attacks only in the fall or spring, when the rains brought new grass for their animals to graze on. Even at that, they must have used a lot of energy and time finding grass and water for their ponies. With the bounty of the Nile at their disposal, and much shorter supply-lines, the Mamluks would have been able to bring grain and hay to supplement the sparse pastures of the Holy Land.
